16: Account not valid in this region (iOS)

Means
On an iPhone or iPad, Hulu's servers flagged your IP address as coming through an anonymous proxy or VPN. Hulu's content rights cover only the United States, so it blocks those connections.
First step
In Settings > Wi-Fi, open the details of the connected network and set HTTP Proxy to Off. All steps
PB3: Outdated video or audio drivers (Windows)

Means
The Hulu app for Windows is missing the latest video and/or audio drivers. Hulu requires Microsoft-signed drivers to protect your device and its content.

Means
Hulu says this usually comes from outdated drivers, running Windows in a virtual machine such as VMware or Parallels, or running Windows on a partitioned computer like a MacBook.
